Inquiry into patient's suicide

=Mr Wells was being treated after trying to kill himself earler this month-tv-Mr Wells: http://news.bbc.co.uk/1/hi/england/south_yorkshire/7529786.stm!!bbc,-tv-Michael Wells, 60, was being escorted by a female carer in a taxi when he escaped and seemed to throw himself under a lorry in Barnsley on 11 July.-tv-His wife Ann, of Gerald Place, Kendray, Barnsley, said his death was avoidable and she is considering legal action.-tv-Barnsley Primary Care Trust (PCT) said an investigation was under way.-tv-Mr Wells was a voluntary psychiatric patient being transported from Barnsley Hospital’s Oakwell Centre to the town’s Kendray Hospital in a taxi, accompanied by a nurse, when he jumped out and ran off on 11 July.-tv-He was first hit by a car before running on to Huddersfield Road where he appeared to throw himself under a lorry.-tv-'Suicide watch'-tv-Mr Wells had to have both legs amputated and died in hospital last Wednesday - 12 days after the incident.-tv-His widow told the BBC: 'They knew he was suicidal. He had been on suicide watch down there night and day.'-tv-'I don’t understand why they transferred him in a taxi to the hospital. It should have been an ambulance.'-tv-She believes if he had been taken by ambulance, with a two-man crew, her husband would never have been able to escape.-tv-'I am left with no husband, I have to bring up the grandbairns [grandchildren] without their grandfather now and I can’t cope,' she said.-tv-A Barnsley PCT statement said: 'It is the policy of Barnsley PCT and the Department of Health not to comment in the media about individual patients and to do so would be to compromise patient confidentiality.'-tv-'Investigations into the incident are currently under way. Until it has been thoroughly investigated the PCT cannot comment further.'-tv-Meanwhile, an inquest into Mr Wells’s death has been opened and adjourned.-tv-
